29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Work Station #31;  A white surge protector was plugged into another white surge protector. The UL listing states that relocatable power taps (surge protectors) are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.    (b)  Work Station#14;  A Heat Stream brand heater rated at 12.5 amps was plugged into a surge protector rated at 12 amps. The UL listing states that relocatable power taps (surge protectors) are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    The Internal Revenue Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 962602, citation number 1, item number 1 and was affirmed as a final order on April 18, 2014, with respect to a workplace located at Bellingham, WA.  The Internal Revenue Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 428412, citation number 1, item number 1 and was affirmed as a final order on August 14, 2012, with respect to a workplace located at Seattle, WA.  The Internal Revenue Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 425545, citation number 1, item number 1and was affirmed as a final order on November 7, 2012, with respect to a workplace located at Seattle, WA.
