29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a) Throughout Facility - On or about June 22, 2015, Employees were required to change cutting knifes and / or make necessary repairs / adjustments on hard-wired machines daily. The employer's written Lockout / Tagout program failed to identify specific steps for shutting down or isolating each and every energy source on every piece of equipment. Employees were thereby exposed to the hazards of the unexpected reenergization of equipment.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Throughout facility - On or about June 22, 2015, the employer had not performed periodic inspections at least annually since 2012, to ensure that documented lockout/tagout procedures were being followed by authorized employees.
