OSH ACT of 1970 Section (5)(a)(1): the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees riding an excavator bucket out of a trench were exposed to fall and/or struck-by hazards:   a) 1200 Pinnacle Pkwy., Covington, LA; On or about May 21, 2015, the employer allowed employees to ride the bucket of a Case (Model CX160) hydraulic excavator to egress a trench approximately 12 feet in depth, exposing employees to fall and struck-by hazards.
