29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:    On or about May 18, at the job site located at 603 N Monitor Rd in Springdale, Arkansas, employees were required to enter spaces, which met the definition of permit required confined spaces, to clear jams and repair plastic recycling equipment. The employer had not evaluated the spaces to determine if permits were required for entry. These conditions exposed the employees to the hazard of being caught in the equipment in the following instances:    a) The conveyor and chute above the Kowin model 346 plastic shredder  b) The conveyor and chute above the Kowin model 378 plastic grinder  c) The heating tank  d) The separation tank  e) Ground plastic storage tank 1  f) Ground plastic storage tank 2  g) Pellet storage tank

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about May 18, 2015, at the job site located at 603 N Monitor Rd in Springdale, Arkansas, employees were required to perform maintenance and clear jams on plastic recycling equipment under lock out/tag out conditions. The employees did not have procedures available, outlining the steps for the orderly shut down, isolation of energy sources, and application and removal of lock out devices for the machines. These conditions exposed the employees to the hazard of being caught in the machines in the following instances:    a) Belt conveyor feeding  the shredder  b) Kowin model 346  plastic shredder   c) Belt conveyor on discharge side of the grinder  d) Belt conveyor feeding the grinder  e) Kowin model 378 plastic grinder  f) Screw conveyor on discharge side of grinder  g) Screw conveyor feeding the 4 in 1 wash system  h) Kowin model 196 4 in 1 wash system  i) Centrifugal drier  j) Air separator  k) First vibrating screen separator  l) Screw conveyor feeding the extruder  m) Kowin model 272 pellet extruder  n) Drier on discharge end of cooling system  o) Second vibrating screen separator

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    On or about May 18, 2015, at the job site located at 603 N Monitor Rd in Springdale, Arkansas, employees performing maintenance work on plastic recycling machinery were required to work from a forklift mounted platform that had not been approved by the forklift's manufacturer. These conditions exposed the employees to the hazard of falling approximately 8 feet to the concrete floor below.
