29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: (a) Receiving dock -- On or prior to 5/14/15, a safe aisle width was not maintained between the downstroke baler positioned between dock doors 2 and 3 and the PET blower platform. On 5/14/15, an employee operating a down stroke baler was severely injured when he was struck by a forklift as the forklift operator attempted to pass by. The aisle width was restricted by the location of the baler and bags of recycling near the PET blower platform.

(c) Nissan "white" lift truck model MP1F2A25LV, Chassis no. P1F2-9H101: On or prior to 5/19/15, the lift truck had been in service with "no mirrors" noted by the operator as defective. (d) Yale "clam" lift truck model ERP040VFN36TE082, SN A955N01539H: On or prior to 5/19/15, the lift truck was in use with hoses, lights, steering, and tires noted by the operator as defective.
