29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: a. In the Carpenter Shop's at the eight-inch Jointer Machine - On or about May 14, 2015, an employee was feeding a wedge shaped piece of wood material into the machines cutting surface when the wedge kicked-back, flipped over and resulted in the employee sustaining a partial amputation to the left hand ring finger.

29 CFR 1910.1030(g)(2)(ii)(A): The employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place: a. On or about May 14, 2015, in the Carpenter Shop, an employee assigned to clean a jointer machine that was contaminated with blood or other potentially infectious materials was not trained by the employer to perform this task potentially exposing the employee to bloodborne pathogen hazards.
