29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:  In the spraying area, on or about May 22, 2015 and at times prior thereto and thereafter, trash was allowed to accumulate after bedliner spraying operations were discontinued, including, but not limited to, soiled shop towels, used sanding discs, overspray-coated cans of flammable chemicals including Klean-Strip Denatured Alcohol and Advantage 105 Fast Dry Economy Lacquer Thinner, flammable aerosol cans including BT50 Black Primer, and combustible and ignitable waste including the cardboard backsplash and overspray residue on fan blades, exposing employees to the hazards of fire.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  In the shop, on or about May 22, 2015 and at times prior thereto and thereafter, where employees were using flammable aerosols including, but not limited to, WD-40, and were storing flammable chemicals including, but not limited to, AutoBahn AP-8040 Euro Prime English White primer and aerosol Blaster PB Penetrating Catalyst, the side-hinged swinging exit door used to access the west side of the property, where the spray booth was located, was not posted with an Exit sign, exposing employees to the hazards of fire.

29 CFR 1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:  a) On the east wall of the shop, access to the Siemens breaker box controlling electrical equipment including lights, heaters, microwave oven, 5-ton A/C, and water heater was blocked by common combustibles including stacked lumber and cardboard boxes, and truck accessories including a toolbox and front end grill, exposing employees to the hazards of fire.   b) On the west wall of the shop, access to the breaker box controlling electrical installations and equipment including the automotive lift, wall outlets, and the HVAC system was blocked by a workbench with flammable chemicals stored on top and five truck tires in front, exposing employees to the hazards of fire.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At the inspection site, on or about May 22, 2015 and at times prior thereto and thereafter, the employer did not develop and implement a written hazard communication program for employees who were using hazardous chemicals including, but not limited to flammable WD-40 and CarQuest Wearever Brake Cleaner containing perchloroethylene, which is classified as probably carcinogenic to humans (Group 2A) by the International Agency for Research on Cancer (IARC), exposing employees to the hazards of chemical contact and/or inhalation and fire.   The Centx Custom Truck Accessories was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 1016586, Citation number 1, Item number 3a, and was affirmed as a final order on March 18, 2015 with respect to a workplace located at 104 Enchanted Cove, Cedar Creek, Texas.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:  a) In the shop, the upstairs parts storage area for appliance parts was not posted with the floor load limit for employees who were using office areas underneath, exposing employees to the hazards of contusions.
