29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) 503 Newburg Road, Shippensburg, Pa - The employer did not ensure that employees working on the 4th floor with an open side were protected from falls with guard rails, safety nets or personal fall protection, thereby exposing employees to an approximately 20 foot fall hazard on or about April 16th, 2015.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a) 503 Newburg Road, Shippensburg, Pa, Roof access, South side - Employees  working on the roof were exposed to a approximated 32 inches by 32 inches unprotected hole, on or about April 16th, 2015  Poole Anderson was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(ii), which was contained in OSHA inspection number 112989884 citation number 1,  item 1, and was affirmed as a final order on September 29, 2010 with respect to a workplace located at University Drive & Park Avenue, University Park, Pa.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
