29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: (a) On or about May 16, 2015, an employee was seriously injured from a load being lifted by a Demag 2 ton overhead crane, serial number 87851, utilizing a single choker sling which was not placed on the center of gravity of the load. This created an unbalanced load, resulting in the load shifting during the lift.
