On 5/13/15 an employee was polishing part #HAL00024160 and contacted one of the ingoing nip points resulting in an amputation of the tip of his right thumb.      b)  Glass Bead Booth, Work Cart - The idler shaft, which rotates due to contact with the part being worked, was not guarded.
