29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Glass Bead Booth, Work Cart - The ingoing nip points at the interface of the cylindrical part being polished and the eight wheels of the work cart supporting the part were not guarded. On 5/13/15 an employee was polishing part #HAL00024160 and contacted one of the ingoing nip points resulting in an amputation of the tip of his right thumb. b) Glass Bead Booth, Work Cart - The idler shaft, which rotates due to contact with the part being worked, was not guarded.
