29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(C)(4):               a) On or about May 18, 2015, at the above located workplace, employees were required to operate, unjam and perform maintenance on equipment such as bailers and a conveyor belt. The employer did not establish a written lockout/tagout program. The employees were thereby exposed to the hazards associated with the unexpected energizing or startup of equipment.

29 CFR 1910.176(b): Storage of material created a hazard.   a) On or about May 18, 2015, at the door just north of the large baler, where employees were required to work, there were two compressed gas cylinders without chains or other means to prevent them from falling. Employees were thereby exposed to the hazards associated with unchained compressed gas cylinders.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by nip points:                                  a) On or about May 18, 2015, at the above located workplace, employees were required to work with a large horizontal baler. The employees were exposed to large nip points between the unguarded moving hydraulic ram and the sides on the baler.  The employees were thereby exposed to hazards associated with nip points.          b) On or about May 18, 2015, at the above located workplace, employees were required to work with a large horizontal baler. The employees were exposed to nip points between the unguarded guide wheel and the rail on top of the ram's hydraulic cylinder. Employees were thereby exposed to hazards associated with ingoing nip points and rotating parts.

29 CFR 1910.219(f)(3): All sprocket wheels and chains that were less than seven (7) feet above the floor or platform were not enclosed:   a) On or about May 18, 2015, at the above located workplace, employees were required to work adjacent to a balers feed conveyor belt. The conveyor belt had an unguarded drive chain and sprocket which was approximately six (6) feet above ground level. The employee was thereby exposed to the hazards associated with moving parts.
