29 CFR 1910.263(e)(1)(viii): Employer did not ensure that every mixer was equipped with a full enclosure over the bowl which is closed at all times while the agitator is in motion: (a) Steven Roberts Originals LLC, at 2780 Tower Road, Aurora, CO: On 5/26/15 and preceding the employer did not ensure that every mixer was equipped with a full enclosure over the bowl which is closed at all times while the agitator is in motion. An employee used a Hobart model V1401 horizontal mixer to mix dough in the cake pop mixing area of the Finishing Department. The guard was not closed during operation. This condition exposed the employee to an amputation hazard. (b) Steven Roberts Originals LLC, at 2780 Tower Road, Aurora, CO: On 5/26/15 and preceding the employer did not ensure that every mixer was equipped with a full enclosure over the bowl which is closed at all times while the agitator is in motion. An employee used a Hobart model V1401 horizontal mixer to mix dough in the mixing area of the Finishing Department. The guard was damaged and non-functional. This condition exposed the employee to an amputation hazard.

The flexible power cords supplying power to two Rocky Mountain Portable Cooling Inc cooling units and a fan were plugged into a drop pendant box. The ground pins for the power cords were missing. This condition exposed employees to an electrical hazard.

The flexible power cord supplying power to a Rocky Mountain Portable Cooling Inc cooling unit was damaged in proximity to the plug at the drop pendant box. The conductors of the live circuit were exposed. Support had not been provided to the flexible power cord to prevent damage. This condition exposed employees to an electrical hazard.
