29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Balcony: On or about May 13, 2015, the employer did not ensure fall protection was used while working near the balcony's unprotected edge which exposed employees to a fall of 27-feet to the ground.

29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) Balcony: On or about May 13, 2015, the employer allowed the anchor device to be installed and used while attached to the mullion of the window which exposed an employee to a fall of 27-feet to the ground.
