29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about 05/26/2015 employees at the Nelson Street roofing project were exposed to approximately 40' falls. Employees were engaged in roofing activities near an unprotected skylight. The Sky Blue Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 982996, citation number 01, item number 01, and was affirmed as a final order on 09/19/2014, with respect to a workplace located at 412 Farmington Avenue, Hartford, CT 06105.
