29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the return motion of the winder. On or about May 16, 2015, the company failed to protect employees from machinery hazards. While working on an unguarded winder machine, an employee suffered an amputation to his right index finger.
