OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards when the foot control pedal on the Wisconsin Machine Corporation HD-10 mechanical press brake (SN: 69159) was not guarded from unintended or inadvertent operation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  WEST WALL, PRODUCTION AREA - Employees used the Wisconsin Machine Corporation HD-10 Press Brake, and the press brake was not equipped with any type of guard or system to prevent access to the point of operation while the machine was being operated, thereby exposing employees to possible amputation and crushing injuries, on or about April 22, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:    (a)  COLUMN 6, PRODUCTION AREA - Employees used a Millermatic 252 Welder, and the welder was plugged into a homemade extension cord where the flexible part of the cable contained a splice that had been joined using wire nuts and electrical tape, thereby exposing the employee(s) to electrical shock and fire hazards, on or about April 22, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    (a) PRODUCTION AREA, SOUTH WALL - Employees stored and retrieved parts from a parts rack located adjacent to the South wall in the production area, where a junction box mounted on a column, did not have a cover installed.  The live conductors routed through the junction box were protruding severely through the front of the junction box, thereby exposing the employee(s) to electrical shock and fire related injuries, on or about April 22, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a)  WEST WALL, PRODUCTION AREA NEAR PRESS BRAKE - Employees worked in the production area near the Wisconsin Machine Corporation Press Brake, where the strain relief on the power cable exiting the bottom of the electrical disconnect supplying electricity to the press brake had been pulled away, thereby exposing employee(s) to electrical hazards, on or about April 22, 2015.    (b)  NORTHWEST CORNER PRODUCTION AREA, AT THE CINCINATTI SHEAR - Employees worked in the production area near the Cincinnati Shear, where the strain relief on the power cable entering the shear had been pulled away, thereby exposing employee(s) to electrical hazards, on or about April 22, 2015.    (c)  COLUMN 5, EAST SIDE - Employees worked in the production area near column 5, where the strain relief on the power cable exiting the bottom of the electrical disconnect supplying electricity to a Millermatic 252 welder had been pulled away, thereby exposing employee(s) to possible electrical hazards, on or about April 22, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
