29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations: a. Site - Employees performing demolition work on a two story building were not protected from a wide range of hazards associated with the demolition of a structure when the employer did not have an engineering survey performed by and put in writing by a competent person prior to the start of work. The failure to conduct the engineering survey contributed to a Cat 330 excavator falling through a hollow core pre-stressed concrete slab deck and a masonry wall collapsing into a public road.

29 CFR 1926.856(a): Floors or working surfaces were not of sufficient strength to support the imposed load of mechanical equipment: a. Site - on 5/18/2015 a subcontractor was injured when the Cat 330 excavator he was using to demolish a building was positioned on a hollow core pre-stressed concrete slab deck which extended beyond the main structure and contained a portion of the basement below resulting in the collapse of the concrete slab beneath and subsequent fall of approximately 10 feet for the excavator and operator within the cab.
