29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed. Employees were exposed to caught in and amputation hazards: a. On the dock-door side of the production area, the large oven had a large sprocket wheel and chain and two conveyor sprockets that were not guarded or enclosed; b. In the production area, a remover cutting machine had a sprocket wheel and chain that was not guarded or enclosed.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. In the corner of the facility behind the winding machines, electrical equipment was blocked and the space was used as storage by a tool cart and rolls of insulation exposing employees to electrical shock and fire hazards.
