OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 SECTION (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: On or about May 18, 2015, employees were exposed to fire and explosion hazards while vacuuming out crude oil from the "gun barrel" oil water separator located 4 miles north of Highway 36 on County Road 7, northwest of St. Francis, Kansas. The screw plug immersion heater was not locked out and de-energized. Among other methods, feasible and acceptable methods is to lockout the heater prior to working on the "gun barrel" oil water separator. American Petroleum Institute Recommended Practice 54 (API/RP 54), Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations March 2007 edition states in Section 6.9.1(a) Locks and/or tags should be placed to plainly identify the equipment or circuits being worked on."
