29 CFR 1910.106(e)(2)(iv)(a): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), must be kept in covered containers when not actually in use.  (a) Chemical storage area: The employer failed to maintain category 2 flammables (SS-160) covered when the containers were not in use. Employees were exposed to inhalation and ingestion hazards during the transfer of SS-160 from the 55 gallon drum to the small tank for use.

29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), must not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section must be deemed to have been complied with.  (a) Chemical Storage area: The employer failed to protect employees from electrostatic hazards during the unsafe transfer of flammable liquids from one 55 gallon drum into a smaller tank such as but not limited to SS-160 (Aliphatic Naphtha). Employees were allowed to transfer flammable liquids without the containers being properly grounded and bonded.

29 CFR 1910.106(e)(9)(ii): Adequate aisles must be maintained for unobstructed movement of personnel and so that fire protection equipment can be brought to bear on any part of flammable liquid storage, use, or any unit physical operation.  (a) The chemical storage area next to the wash department. The employer failed to provide a clear walkway with unobstructed movement for employees to enter and exit the storage area during the transfer of chemicals from one 55 gallon drum to a 5 gallon drum. The storage area of obstructed by parts, boxes, and other 55 gallon drums not yet placed on pallets. Employees were exposed to obstructed walkways causing difficultly getting to the chemicals and it spills occur no place to go in order to get out of the pathway of the chemical.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  (a) Ironform: The employer failed to report an inpatient hospitalization to OSHA with 24 hours. On January 12, 2015 an employee was taken to the local emergency room and then transported to Mercy Hospital in Springfield for a lacerated spleen after being struck-by a part in the upper abdomen.
