29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Commercial building: On or about May 26, 2015 and at times prior thereto, two employees working on roof were using a ladder for access and egress on the north side of the building that was not extended the required 36 inches above the roof eave. This condition exposed the employees to a fall greater than 6-feet to the ground below.
