29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about 5/18/2015,at the jobsite the employer allowed an employee to perform decommissioning work inside the trailer of an 18 wheeler semi. The employee accessed the trailer through an unguarded opening in the side of the trailer that was greater than four feet above the ground.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: On or about 5/18/2015 at the jobsite the employer allowed an employee to utilize a set of five riser metal stairs to access the side opening on the trailer. The metal stairs did not have a railing on each side.
