29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about 06/26/15 located at 4120 Creststone Ct., Cumming, GA:  Employees were observed installing floor joist on to the 2nd floor of a house that was under construction with a pneumatic nail gun and using a skill saw without safety glasses.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 06/26/15 located at 4120 Creststone Ct., Cumming, GA:  Employees were exposed to14.5 falls and struck- by hazard, while preforming framing work from the make shift walking/ working surfaces. The employer failed to install guard rails and it was not fully decked.

29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   On or about 06/26/15 located at 4120 Creststone Ct., Cumming, GA: Employees were framing and installing the 2nd floor, floor joist of a house that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 14.5 feet.
