29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, the metal electric box of a phone buzzer was disconnected from the wall, hanging from the power cord, and incorrectly wired to the power circuit. b) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, a metal electric switch box was disconnected from the wall, separated from the conduit connections, and hanging from the inner wiring electrical cords. c) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, a metal electric key switch box was disconnected from the wall. d) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, a four inch square metal knock out type box with an alarm bell was disconnected from the wall, separated from the conduit connections, and hanging from the inner wiring electrical cords.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable trays, cable armor, cable sheath, enclosures, frames, fittings, and noncurrent-carrying parts that are to serve as grounding conductors, with or without the use of supplementary equipment grounding conductors, were not effectively bonded where necessary to ensure electrical continuity and did not have the capacity to conduct safely any fault current likely to have been imposed on them: a) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, the metal electric box of a phone buzzer was disconnected from the wall, hanging from the power cord, and not effectively bonded to ensure electrical continuity. b) Quad Graphics Inc., Baler / Shredder Room - On or about May 27, 2015, a metal electric switch box was disconnected from the wall, separated from the conduit connections, hanging from the inner wiring electrical cords and was not effectively bonded to ensure electrical continuity.
