29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (a) Dawn Food Products, Inc. @ 4500 Lipan Street, Denver CO 80211: On or before May 27th 2015, the employer did not perform periodic inspections to identify discrepancies within the lock-out tag-out (LOTO) procedures which exposed affected employees to amputation hazards. On May 18th 2015 an employee attempted to take a swab sample from the double-head pail filler machine, which was operating in the "Clean in Place" mode, and suffered a finger tip amputation. Affected employees had performed this task on many occasions when the machine was shut-down for manual cleaning. This condition exposed employees to amputation hazards.

29 CFR 1910.147(c)(7)(i)(B): Each affected employee shall be instructed in the purpose and use of the energy control procedure. (a) Dawn Food Products, Inc. @ 4500 Lipan Street, Denver CO 80211: On or before May 27th 2015, the employer did not provide training to all employees who come into contact with automated machinery during routine duties. Quality-Assurance personnel routinely relied on machine operators to tell them if a machine was powered down and safe to examine, rather than being trained to recognize the hazards and/or use of lock-out tag-out (LOTO). This condition exposed employees to amputation hazards from energized equipment.
