29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Shop Area - An employee used a Peddinghaus 140 hydraulic punch to punch holes in plates of metal but the point of operation was not completely guarded, thereby exposing the employee to a serious hazard, observed on or about 5-27-15. Abatement certification required.
