29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about May 20, 2015, and at times prior there to, an employee was operating a mechanical punch press machine that had no guarding near the point of operation. The employee was exposed to an amputation, struck-by, and caught-in-between hazards.
