29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At 111 East Liberty Street in Farmington, MO - On or about 05/26/2015, employees were working on a steep roof (5 in 12 pitch or greater) approximately 16 feet above a lower solid concrete/asphalt surface without having any fall protection system in place, exposing employees to elevated fall hazards. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 111 East Liberty Street in Farmington, MO - On or about 05/26/2015, employees were using an aluminum extension ladder to access a roof top, approximately 12 feet above a lower solid concrete/asphalt surface where the ladder side rails only extended 1 and ½ feet above the upper landing surface being accessed, exposing employees to elevated fall hazards during ladder to roof, or roof to ladder, transfer.
