29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a)  At 2453 State Highway H in Sikeston, MO 63801 - On or about 05/27/2015, employees were working with pneumatic tools (nail guns), handling splintered wooden pallets, and operating machinery, such as the de-nailing pallet cutter, necessitating the use of eye, hand, and hearing protection, and the employer had not assessed the work area to determine the use of personal protective equipment.

29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:   a)  At 2453 State Highway H in Sikeston, MO 63801 (Repair Shop) - On or about 05/27/2015, employees were using a restroom that was unsanitary and where the toilet had a broken ceramic cover, exposing internal toilet parts, and where remnants of defecation and urination were present on and around the toilet, exposing employees to contact with bacteria hazards.
