This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s)created by point of operation, rotating parts and pinch points: (a) Broaching Department : The employer did not ensure that all broaching machines throughout the facility were equipped with machine guarding to protect employees from pinch point hazards. (b) Cutting Department : The employer did not ensure that the un-used portion of the blade on the HA-250 vertical band-saw was guarded to protect employees from the blade while in operation. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. 29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable. (a) Broaching Department - The employer did not ensure that the pit used to contain the excess oil from the broaching machines were equipped with platforms for employees to walk on when they were required to clean out the back of the machines. Skids were placed in the pit for employees to stand on to access the area. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification.
