29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a) 2900 Fulling Mill, Road, Middletown, PA / Construction area main entrance door; A 110 volt extension cord used to power a Dewalt 12 inch Chop Saw was passing through a metal doorway and not protected from damage, on or about 05/28/2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
