29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about May 13, 2015 in the fabrication department, the hold-down plate on the end-bender was not equipped with a guard. Employees operating the end-bender were exposed to the hazard of their fingers being caught between the hold-down plate and the panel.
