29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):  Worksite located at 2700 E Hwy 33. Guthrie, OK 73044. On or about May 28, 2015, adequate protective systems, such as benching, sloping, or shielding were not used to protect employees working in the trench. The vertical wall of the bench exceeded the 4 foot maximum, exposing employees to engulfment or crushed-by hazards.
