29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a) Facility Wide - The employer did not ensure that machine operators and technicians were adequately trained as "Authorized" employees and/or knowledgeable in the recognition of minor servicing procedures that require machine or equipment shutdown and lockout to be performed to prevent injury. Machine operators and technicians perform servicing tasks, including conducting change overs or setups, and clearing jams.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6):  a) Torsional Department and Steel Blanking Department: The employer did not ensure that employees performing change over or setup of dies and clearing jams on machines including the Torsion 4212  and the Minster Press 41 were required to:   (1) prepare the machine or equipment for shut down,  (2) turn off and shut down the machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source(s),  (4) affix lockout or tagout devices to each energy isolating device, and (5) verify isolation and deenergization of the machine or equipment have been accomplished prior to starting service and/or maintenance work.  Employees were exposed to the hazard of being caught-in from the unexpected start-up and energizing of the machine.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device before working on the machine or equipment:    a) Department 28 Ross 2/Slitter Area- The employer did not ensure that each employee in a group performing work on machines or equipment, including setups and preventive maintenance on the Elite Cameron Slitter 265, affixed their personal lockout device as a part of a group lockout.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    a) Area 131, Torsion Department- The employer did not ensure that guards on machines, such as the Torsion 4212 conveyor access guard, were affixed or secured in a manner that would prevent employees from having any part of their body in the danger zone during the operating cycle.  On or about May 22, 2015, an employee sustained an amputation injury when reaching in through the opening in the conveyor access guard on the Torsion 4212.
