29 CFR 1910.28(j)(4): The workman shall be protected by a safety life belt attached to a lifeline. The lifeline shall be securely attached to substantial members of the structure (not scaffold), or to securely rigged lines, which will safely suspend the worker in case of a fall. a. On May 28, 2015, at 51 North 3rd Street, Newark, Ohio, Two employees who were conducting window cleaning operations on a seven story building, utilizing a boatswains chair descent system, connected their fall arrest systems, boatswain's chair, and tie-back for the mule to a water pipe that was used as a single anchor point. Independent anchorages were not used for the personal fall arrest system, suspension system, and an engineering survey was not completed on the water pipe to ensure it was capable of supporting the loads with a 4 to 1 safety factor, thereby exposing the employees to a 70 foot fall hazard.
