OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck by and crushing hazards related to hoist and spreader bar operations:    Location - Pouring Area:  a. On or about June 11, 2015, employees were pouring molten materials into a mold using a damaged hoist that had not been inspected.  Employees were exposed to crushing injuries as they moved the hoist and attached ladle holding the molten material around that area of the facility.  Other employees were also exposed while they standing next to the hoist while they were preparing to move the molds once the pour was complete.    Location - Shakeout Pit Area:  b. On or about June 11, 2015, employees were using a damaged hoist system that had not been inspected or rated.  Employees were exposed to crushing hazards as they moved the hoist and attached bucket holding up to 700 pounds of sand.  The carrying bucket for the sand was distorted and cracked form the weight of the sand over time.    Location - Sand System Area:  b. On or about June 11, 2015, employees were exposed to struck by and crushing hazards from falling loads, in that, a spreader bar in use was not inspected or tested.

An employee was performing pouring/shifting duties of molten iron which is capable of causing splashing burns and was not required to wear protective gear to prevent burns to arms, legs or torso.
