29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Facility - On or about April 28, 2015, an employee operating a Star SH-16 CNC machine experienced a work-related amputation of the left index finger. The employer reported this work-related amputation to OSHA on or about May 22, 2015.
