29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a - 634 Union Avenue, Delanco NJ: A company employee was exposed to falls, in excess of 20 feet to the ground surface below, when working from a job made wooden box that sat on the forks of the worksite Lull all terrain forklift and was moved horizontally with the employee standing in it.  This box contained guardrails that did not meet the height requirements of the standard nor did the guardrails totally enclose the exposed employee on all four side.  The exposed employee was not wearing a full body harness nor was he provided with an alternate fall protection means at the time of the observation.  Condition noted by CSHO on or about 05/28/15.
