29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:    a- 634 Union Avenue, Delanco NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall hazards.  Condition noted on or about 05/28/15.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a - 634 Union Avenue, Delanco NJ: A company employee was exposed to falls, in excess of 20 feet to the ground surface below, when working from a job made wooden box that sat on the forks of the worksite Lull all terrain forklift and was moved horizontally with the employee standing in it.  This box contained guardrails that did not meet the height requirements of the standard nor did the guardrails totally enclose the exposed employee on all four side.  The exposed employee was not wearing a full body harness nor was he provided with an alternate fall protection means at the time of the observation.  Condition noted by CSHO on or about 05/28/15.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a- 634 Union Avenue, Delanco NJ: A company employee was exposed to falls, in excess of 20 feet to the ground surface below, when performing roof sheathing replacement and other related activities while at the unprotected edge(s) of a residential roof and while within the residential roof's rafters.  This employee was not wearing fall protection nor provided with an alternate fall protection means at the time of the observation.  Condition observed by CSHO on or about 05/28/15.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a- 634 Union Avenue, Delanco NJ: A company employee was exposed to falls, in excess of 20 feet to the ground surface below, when performing roof sheathing replacement and other related activities while at the unprotected edge(s) of a residential roof and while within the residential roof's rafters.
