29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a. A&D Environmental Services, 100 Waste Research Drive, Macon, GA (Warehouse Area) - An employee was exposed to fall hazard. An employee was walking and working in close proximity to an unguarded/unprotected skylight, when checking the level of a steel support beam on the roof, exposing employee to a fall hazard of approximately 24 feet to ground level, on or about March 8, 2015.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a. The employer learned of the death of an employee on March 13, 2015, as the result of an incident that occurred on March 8, 2015 at the A&D Environmental Services jobsite, and was never reported to OSHA by the employer. The incident was discovered by a complaint which was reported to the Atlanta East Office and was verified upon the inspection on May 20, 2015.
