29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    Employees were exposed to skin contact with Cal Clean 878, alkaline cleaner during mixing activities and during pressure washing in the wash booth and the employer did not provide and require the use chemical protective clothing to prevent contact with the skin.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation had been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  The employer did not evaluate, develop and document a personal protective equipment hazard  assessment for employees exposed to skin contact with Cal Clean 878, alkaline cleaner during mixing activities and during pressure washing in the wash booth nor did they complete a written certification that included the name of the person who certified that the evaluation had been performed and the date that the hazard assessment was completed.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a)  Employees were exposed to contact with caustic substances when they pre-mixed Cal Clean 878, a corrosive alkaline cleaner, and an emergency shower was not provided for emergency use.  b)  Employees were exposed to contact with Cal Clean 878 when they pressure washed parts inside the wash booth and an emergency shower was not provided for emergency use.
