29 CFR 1926.652(e)(1)(ii): Support systems were not installed and removed in a manner that protected employees from cave-ins, structural collapses, or from being struck by members of the support system: Project P-1045, North-South Potable Water Conveyance, on Wire Mountain Road on or about 05/28/2015: The employer did not insure that the cave in protection is fixed and will not move in case of a cave-in. The cave in protection was smaller in size that the excavation.
