29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used. Located on the interior area of the structure there was an electric Makita circular saw being used powered through an extension cord to an exterior 110v temporary electric service. The energized Makita circular saw had damaged primary insulation in at least three locations through the primary insulation to the secondary insulated conductors.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Located along the interior area of the structure there was an employee observed performing framing filler block instillation operations working from an interior stud wall top plate without conventional or alternative fall protection and was exposed to a fall potential in excess of 9.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. Located along the interior area of the structure there was an employee performing framing filler block instillation operations observed gaining access from an interior stud wall top plate work location by climbing the stud wall and stud wall filler block which had a change in elevation in excess of 9 without the use of a ladder or other regulation compliant method being utilized.
