29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  a) 462/464 Smith Street, Perth Amboy, NJ 08861. Employee was exposed to eye and face injuries when operating a pneumatic nail gun while not wearing any eye and or face protection.    Condition occurred on or about 05/30/15.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:  a) 462/464 Smith Street, Perth Amboy, NJ 08861. Employees were exposed to 16 foot, 6 inch fall hazard while working on and from a flat roof installing new roofing materials with no fall protection systems in place.     Condition occurred on or about 05/30/15.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  a) 462/464 Smith Street, Perth Amboy, NJ 08861. Employees were exposed to fall hazards while using a 20 foot extension ladder to access the roof when the ladder did not extend three feet above the landing.    Condition occurred on or about 05/30/15.
