29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Location: Mill On or about 5/22/15 the employer did not adequately control the accumulation of dust from gypsum processing. The dust was over 4 inches deep in several areas including on structural members, electrical disconnects and floors.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Location: zone 2 control station (b) Location: "pulp" area On or about 5/22/15 employees were not protected from electrical shock and burns from energized 480 volt electrical panel boards that were missing the internal trim and accessible to unauthorized employees.
