29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a) A-OK Pools & Spas, at 9081 Federal Boulevard, Denver, CO: On 5/29/15, the employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Employees removed paint and plaster from the surface of an in-ground swimming pool using portable right angle grinders.  The grinding process generated flying particles.  Two employees wore non-safety sunglasses.  This condition exposed employees to a flying particle hazard.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:    (a) A-OK Pools & Spas, at 9081 Federal Boulevard, Denver, CO: On 5/29/15, the employer did not ensure that abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel.  Employees removed paint and plaster from the surface of an in-ground swimming pool using Makita model GA7021 and Ryobi model AG4031G portable right angle grinders.  The right angle grinders were not provided with safety guards.  This condition exposed employees to a flying projectile hazard.
