Employees working on unit number 21 were exposed to caught in hazards from an unguarded sprocket wheel and chain approximately 2 feet from the floor.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) Johns Manville, Waterville OH: on or about Aug 20, 2015, the employer failed to ensure that projecting shaft ends on the unit number 21 were guarded or that the shaft ends did not project more than one half the diameter of the shaft. Shaft ends on unit number 21 pickup roller projected out 2 inches with a diameter of .5 inches exposing employees to a caught by hazard. b) Johns Manville, Waterville OH: on or about Aug 20, 2015, the employer failed to ensure that projecting shaft ends on the unit number 21 were guarded or that the shaft ends did not project more than one half the diameter of the shaft. Shaft ends on unit number 21 brush roller projected out 3.25 inches with a diameter of 1.25 inches exposing employees to a caught by hazard.
