29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose: a) Building 669 South wall area. On or about 5-18-2015, employees were using a triple receptacle temporary power cord for portable tools that was connected through an extension cord, instead of using the triple connected to a dedicated outlet as required by the product listing and labeling. Note: Abatement certification is required for this item.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: a) Building 669, South wall area. On or about 5-18-2015, employees were powering electric screw guns with an power cord that did not have a GFCI or was part of an assured grounding conductor program. Note: Abatement certification is required for this item.
