29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:  a) Third Floor - An employee was observed using an Hatachi pneumatic nail gun while attaching wood framing members and not wearing eye protection, on or about 05/2202015.  b) Third Floor - An employee was using a Makita portable electric saw, model number 5007F to cut wood framing members and not wearing eye protection, on or about 05/22/2015.  c) First Floor - An employee was using a Makita portable electric saw, model number 5007F to cut wood framing members and not wearing eye protection, on or about 05/22/2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Second Floor - A laborer was observed standing below a wood framing operation not wearing head protection and exposed to being struck by falling framing material and or tools, on or about 05/22/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Second Floor, Top of Stairway - Employees were observed stepping over a 30 inch by 30 inch floor hole at the top of the stairway and exposed to an 11 ft. 7 inch fall hazard, on or about 05/22/2015.  b) Second Floor Rear - An employee was observed standing at a 6 ft. wide wall opening and exposed to a 15 ft. fall hazard, on or about 05/22/2015.  c) Third Floor Front - Employees were observed installing wood framing members near the unguarded front and exposed to a 25 ft. fall hazard, on or about 05/22/2015.
