29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Large Allegheny Paper Shredder: Employees were exposed to amputation hazards as lockout tagout procedures were not being utilized for the control of potentially hazardous energy when employees were engaged in unjamming the Large Allegheny Paper Shredder. On or about May 21, 2015, an employee received crushing injuries to their arm and fingers when pulling out paper wrapped around a roller for the Large Shredder Conveyor and lockout tagout was not utilized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Shredding Area: Employees were exposed to amputation hazards as the rotating roller for the conveyor belt was not adequately guarded on the Allegheny Paper Shredder also known as the Big Shredder. On or about May 21, 2015, an employee received crushing injuries to their lower arm and fingers from being caught in the roller on the Big Shredder Conveyor Belt.
