29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, and extremities, respiratory devices, and protective shields and barriers, were not used: On or about June 1, 2015, north side of the Lube N Go Building, an employee was observed cutting out a 6-foot x 6-foot section of asphalt using a Husqvarna K760 14-inch rapid cut saw (5400 RPMs). The employee was not wearing safety rated eyewear, hand or respiratory protection. The employee was exposed to impact/struck-by and inhalation hazards.
